We charge $100 for new patient intake and $75 for follow-up visits.

We charge $75 for DOT Exams.

We charge $250 for Immigration Services(Physical Exams).*

NOTE: Exceptions may apply

* - Fee is only for visit wtih provider and does not include additional services such as labs or x-rays.
